The Staff

North Brook Heights offers a quality living environment managed by a caring, conscientious, and dedicated staff. Our home retains Registered Nurses, Licensed Practical Nurses, Certified Nurses' Aides, an Activity director, and a consulting Registered Dietician. Together the staff functions to meet the health, personal care, and recreational needs of each resident.

The Facility

North Brook Heights is a community based adult home, providing room, meals, housekeeping, personal care, and 24-hour supervision in a warm, friendly, and quality living environment. Licensed by the New York State Department of Social Services, Northbrook Heights is associated with ALP.

The Assisted Living Program (ALP) is a 56 bed facility which is licensed by the N.Y.S. Department of Social Services and the N.Y.S. Department of Health. The ALP is an entity established and operated for the purpose of providing long or short term residential care, room, board, supervision, and providing or arranging for home health services.

The following is a criteria for ALP... Individuals who:

Require more care and service to meet their daily health or functional needs than can be provided by an Adult Care Facility
Are medically eligible for placement in a residential health care facility
Have a stable medical condition
Are able, with direction, to take sufficient action to assure self-preservation in an emergency
The ALP CANNOT provide services to individuals who:

Require continual nursing or medical care
Are chronically bed or chair-fast, requiring lifting equipment or 2 persons to transfer
Are cognitively, physically, or medically impaired to a degree that their safety, or the safety of others, would be endangered.

Rooms

Rooms are designed for single or double occupancy with private bath. Tasteful furnishings are provided, however residents are encouraged to arrange and decorate their own rooms.

All bedrooms are equipped with an emergency call system.

Meals

Three nutritionally balanced meals, plus between-meal snacks, are served daily in a pleasant, central dining room. Accommodations are available for individuals requiring special diets. Special meals are planned for the holidays. Our dietary department provides refreshments for special entertainment events and monthly resident birthday parties.

Personal Care

Personal Care services, including assistance in bathing and dressing, are provided. Barber and beauty services are available in our Beauty Parlor and Barber Shop.

Laundry Service

All linen and personal clothing are laundered by Northbrook at no extra cost.

Health Monitoring

North Brook strives to maintain the optimum health status of each resident. Our RNs and LPNs function to assess each individual's health care needs and coordinate appropriate services and physicians, dentists, medical supply companies, pharmacies, and Public Health nurses. Supervision of medications and scheduled doctor's visits are provided. Transportation for health related services can be provided.

Recreation & Activities

Activities are available daily. All residents are encouraged to participate as well as their families and friends. There are a variety of activities to choose from: movies, arts and crafts, physical fitness classes, library services, bingo, card club, holiday parties, and weekly "happy hour". Religious services for all denominations are held weekly.

Home Care

Our licensed Home Care Agency is staffed with professional and caring employees. An assessment and comprehensive plan of care is developed in conjunction with a registered nurse, patient, and families or planning caregivers. Our dedicated homecare staff includes: Registered Nurses, Licensed Practical Nurses, Home Health Aides, Personal Care Aides, as well as companions. Services may be purchased in hourly time periods starting at 2 hours and up to 24 hours.
